The state Department of Taxation is cautioning Big Island taxpayers that their tax documents may have been lost.

A courier bag was discovered missing Oct. 4 from a Security Courier Service vehicle, department officials said. The company is contracted by the tax department as its courier service.

Officials said about 75 taxpayers may be affected.

The documents lost have been narrowed down to the following:

Business License Forms (Form BB-1) and Cancellation Forms (Forms GEW-TA-RV-1) that were mailed or dropped off between Sept. 24-Oct. 1 at the department’s Kona office.

Tax documents mailed on or about Oct. 1-3, or dropped off on or about Oct. 3-4 at the department’s Hilo office.

The documents include business license applications, tax returns (GET and income tax) with checks, and payment vouchers.

Taxpayers who believe they may be affected are urged to contact the Hawaii District Office at (808) 974-6321.
